Have You Ever Heard Radio Frequency

News by ninechip | On 2019-01-11 09:14
Radio frequency laser is a minimally invasive neuroleptic therapy, which uses controllable temperature to act on nerve tissue to coagulate and desaturate its protein and interrupt the transmission path of pain signal, so as to achieve the purpose of pain relief. Trigeminal nerve is mainly responsible for the sensory function of the face, but there are still some nerve fibers with motor function. So how to destroy only the sensory fibers that transmit pain without damaging the motor fibers? The original denaturation temperature of the two nerve fibers is different, and the sensory fibers are more sensitive, and denaturation can occur at 65 degrees Celsius, while the sensory fibers and the crude fibers conducting touch are relatively late. Blunt, denaturation occurs above 80 C, which gives us a safe temperature range for the treatment of trigeminal nerve rf  tightening treatment. The temperature used in clinic is 70 - 75 C.
Radio frequency thermocoagulation of trigeminal nerve
1. Select the puncture point, usually 1-2 cm away from the corner of the mouth. Disinfect and spread towels.
2. Local anesthesia can alleviate the pain caused by puncture.
3. RF skin machine puncture guided by imaging equipment to the semilunar ganglion in foramen of skull base.
4. Test, because trigeminal neuralgia is the most common single branch pain, so the puncture needle into the foramen also needs to pass sensory test and motor test, further determine the position of the needle tip, fine-tuning to make it located in the root of the branch causing pain. In this process, doctor-patient communication is very important. Patients should accurately describe their feelings after the radio frequency machine emits stimulation, and tell doctors where they feel numb or beating.
5. Intravenous anesthesia, intravenous injection of anesthetic sedative drugs after the completion of positioning, so that patients into sleep, and then give 70-75 degrees C, 120 seconds of Radio frequency ablation, the process is more painful, so patients should be anesthetized. Radio frequency temperature and duration were determined according to the severity of the disease and the characteristics of the branches.
6. The patient wakes up and sticks the puncture wound.
Postoperative precautions
1. After the operation, go to the pillow and lie flat for 48 hours, because the puncture needle will enter the skull base and puncture the dura mater. If the patient sits up too early after the operation, the cerebrospinal fluid will flow out along the puncture needle eye, causing low intracranial pressure headache and other discomforts.
2. Routine use of antibiotics for 3 days to prevent infection.
